Required Profile Options for Form Personalization to work

Following profile options should be set to the user making the Form Personalization:

o ‘FND: Diagnostics’ set to ‘Yes’

o ‘Hide Diagnostics menu entry’ set to ‘No’

o ‘Utilities:Diagnostics’ set to ‘Yes’
